Athletic Forms for Key West High School Athletics
Athletes and Parents – Athletic packets are available in the front office. All students who plan on participating in a sport during the 2017-18 school year, must turn in a NEW athletic packet with a copy of their insurance card, certificate for concussion course, and have a physical on file before starting ANY physical activity. The concussion course can be done by attending one of KWHS's group sessions or individually. If you do the concussion course at home, you must include a copy of the certificate with the athletic packet. Each student will receive a clearance card showing they are cleared through the athletic department when all paperwork is complete. This will be your ticket to start practice. Athletes will turn in the clearance card to their coach when they show up to practice.
2017-18 Athletic packets are not ready yet. They should be available in mid-July.

Please attach a copy of your insurance card to the athletic packet, when it is turned in. If your child doesn't have insurance, you can buy the insurance the school offers. The attached link will help you complete that process.

A physical is must be on file before the start of the season. Physicals last 365 days, so if one was done for one season, it may be able to be used for another sport, depending on the date it was done. The attached link is the physical form that needs to be completed by a physician.
